Nurse Practitioner (Remote, CT License Required)

Galileo is seeking a culturally-competent Nurse Practitioner to join its Population Health team. In this role, you will provide high-quality, comprehensive follow-up care to patients who are post-hospital discharge through phone and video-based care.

Responsibilities:

• Cover care needs for the Transitions of Care program
• Conduct telephonic outreach and scheduled video/phone visits
• Focus on medication reconciliation, symptom screening, and ordering follow-up labs, medications, or supplies
• Navigate conversations with patients to build trust and connect them to services
• Coordinate care across settings including hospitals, EDs, pharmacies, and community providers
• Collaborate with the nursing team
• Be accountable to individual performance targets while contributing to team-level population health goals

Requirements:

• Certified Nurse Practitioner in Adult, Family, or Geriatrics
• Active state licensure in Connecticut (CT), with ability to obtain additional state licenses
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics (2 years may be substituted by Skilled-Nursing facility or Long Term Care experience)
• Direct experience engaging Medicare and Medicaid populations managing complex, comorbid conditions
• Motivational interviewing skills
• Comfort in a digital-first environment
• Solid tech proficiency; familiarity with Google Suite and Slack is a plus
• Excellent written and verbal communication skills
• Bilingual in English and Spanish, including cultural fluency, is a strong bonus

Salary: $75/hour (regular rate) and $85/hour (premium rate)
