Health Information Management (Technician)
Responsible for creating, organizing, coding, and maintaining paper and electronic medical records in compliance with institutional policies and national standards. This role involves accurately logging patient clinical data, coordinating the movement of medical charts, enforcing patient privacy standards, auditing medical files, and collaborating with healthcare professionals to resolve documentation discrepancies.
Responsibilities
- Create, organize, code, and maintain paper and electronic medical records (EMR) in compliance with institutional policies and national standards.
- Accurately log patient clinical data, admission details, diagnostic coding, and discharge summaries into the Health Information Management system.
- Coordinate the prompt movement, retrieval, and filing of medical charts for outpatient consultations, inpatient admissions, and clinical audits.
- Enforce patient privacy standards (e.g., NDPR guidelines) regarding the disclosure, release, and secure storage of health information.
- Regularly audit medical files for completeness, verifying that clinical documentation, consents, and test results are signed and properly filed.
- Collaborate with doctors, nurses, billing officers, and laboratory staff to resolve missing patient documentation or record discrepancies.
Requirements
- Ordinary National Diploma (OND) or Higher National Diploma (HND) in Health Information Management (HIM) or a related medical administration field.
- Registration with the Health Records Officers Registration Board of Nigeria (HRORBN) is required (or actively in progress).
- Minimum of 1–2 years of experience working in a hospital or healthcare facility setting.
- Proficiency in EMR software, database management, and basic Microsoft Office tools (Excel, Word).
- Strong attention to detail, excellent organizational skills, effective communication, and strict adherence to medical ethics and confidentiality.
